Create a site-wide contribute page

Idea is to have a site-wide contribution page which can be used to collect data/data-sets from people. It should contain the following:

1. Some information about the data collection policy, open-ahmedabad etc. How or where this data will be used.

2. A form with the following fields:

Name (Whom should we attribute the data, if any)
Email (Where we can communicate further, if required)
Subject (In short, what is this data?)
Message (Tell us a bit more about the data you are contributing)
Attachment (Only compressed .zip, .rar, .tar.gz files)

Ideally, on submitting the form a new Github Issue should be created and the files stored on a common cloud storage (eg. Dropbox, AWS etc). Or it can simply send the files to an email id accessible to the contributors.

This is almost done, except we're waiting for a commons email ID that the form gets submitted to.
